Universal Transaction Form

In response to feedback from our members, Sunward has created a universal transaction form that can be used for both personal and business transactions.

This paper form is available to members in all drive-up lanes and by request at the teller stations for in-branch transactions. For members who prefer a paperless transaction, it is acceptable to skip the form. Sunward will obtain a signature on the receipt if one is necessary.
